Dear FOI Team, I am writing under the Freedom of Information Act 2000 to request the following information relating to maternity services at Oxford University Hospitals NHS Foundation Trust (OUH), covering the period from 1 January 2020 to 30 June 2024.
1. Maternity Mortality and Morbidity Data Please provide: The total number of births during this period The number and rate (per 1,000) of: Stillbirths Neonatal deaths (0–28 days) Maternal deaths Severe maternal morbidity incidents (e.g. uterine rupture, postpartum haemorrhage >2.5L, eclampsia) Any internal reviews, root cause analyses, or incident investigation summaries related to these outcomes
2. Clinical Governance and Board Oversight A copy of all Board papers or minutes where maternity mortality or stillbirths were discussed (including Quality and Safety Committee meetings) Any action plans or quality improvement initiatives implemented in response to stillbirths or poor outcomes
3. External Reviews and Data Sharing Copies of submissions made to MBRRACE-UK, HSIB, the Perinatal Mortality Review Tool (PMRT), or other external bodies regarding maternal or neonatal deaths during this period
4. Policy and Protocols The current maternal request caesarean section (also known as elective caesarean) policy Any policies or protocols relating to: Elective inductions OXGRIP ultrasound protocol Post-40-week scan provision Any correspondence from the CQC or NHS England relating to concerns about OUH maternity outcomes I request that this information be provided in digital format (PDF or Word).
