Dear Oxford University Hospitals NHS Foundation Trust, I’d like to request the following information under the Freedom of Information Act 2000: The total number of ‘concerns raised’ through the Freedom to Speak Up scheme between 01/09/23 and 01/09/25 (or the most recent month for which data is available), broken down by month. The number of ‘concerns raised’ through the Freedom to Speak Up scheme relating to maternity departments and staff working in maternity departments in the same period, broken down by month. The number of incidents recorded on Datix (or the Trust’s equivalent incident reporting system) between 01/09/23 and 01/09/25 (or the most recent month for which data is available), broken down by month and severity. The number of incidents recorded on Datix (or the Trust’s equivalent incident reporting system) relating to maternity departments and staff working in those departments, for the same period, broken down by month and severity.
