Please provide:
1. The number of conditional employment offers withdrawn because the Trust discovered that an applicant had supplied deliberately false, falsified or fraudulent information.
2. The number of individuals who had already begun working for the Trust when such information was discovered. For both questions, please break the figures down by:
(a) the type of false information, for example qualifications, professional registration, employment history or references; and
(b) the type of professional or position concerned, for example doctor, nurse, physiotherapist, dietitian, healthcare assistant or administrator. If either requested breakdown is unavailable, please provide the closest breakdown held, such as job title, occupation or staff group.
3. For Question 2, Please provide the FIVE longest periods across the three financial years between an individual beginning work and the false information being discovered, expressed in years and days (for example, seven years and four days). If these durations are not held in years and days, please provide them in the closest available unit of measurement. If there were fewer than five such cases, please provide the duration for every case.
Please assign each case to the financial year in which the false information was discovered, regardless of when the application was submitted or the employment began.

We have no identified cases of offers being withdrawn due to deliberately false, falsified or fraudulent information.
2. The number of individuals who had already begun working for the Trust when such information was discovered.
