Please would you give us the number of stillbirths at the JR Hospital maternity unit between 2010 - 2015?
Could you tell us the number of neonatal deaths at the JR maternity unit between 2010 - 2015?
Could you tell us how many of these deaths (stillbirths and neonatal deaths) were infants of mothers referred in from other areas/trusts, and how many were of mothers in the JR's normal catchment area?
How many internal reviews into baby deaths were conducted between 2010 - 2015?
How many of those internal reviews were graded C or D?

We have extracted stillbirth and neonatal death data directly from MBRRACE-UK for the period 2013–2015, which represents the earliest years for which data are held in a consistent, national format that can be reliably reported.

We have been unable to source equivalent data for the preceding period (2010–2012). During those years, maternity governance and reporting arrangements differed significantly from current practice, with information recorded across multiple sources, some of which were paper-based, and without a single consolidated dataset. As a result, the Trust does not hold this historical information in a format that enables reliable retrospective reporting.
In addition, the Perinatal Mortality Review Tool (PMRT), including formal grading of reviews, was not introduced until 2018. PMRT grading is therefore not available for any part of the period covered by this FOI request.
